1. The dog (over 6 months of age) listed on this adoption agreement has been sterilised, unless otherwise advised. If the dog is not sterilised at the date of adoption, I agree to have the dog sterilised at the direction of Staffy and Bully Breed Rescue Inc. (SABBR) or once the dog turns 6 months of age in accordance with the age of the dog stated on this form.

  2. The dog listed on this agreement is up to date with its vaccinations at the date of this agreement. In the case where the dog is under 16 weeks of age, SABBR will cover the first and second compulsory puppy vaccinations, with the third optional puppy vaccination to be at the expense of the adopter.

  3. If the dog listed on this adoption agreement is not registered, I agree to register the dog listed on this agreement with the local shire where the dog resides for the lifetime of the dog in accordance with the Dog Act 1976. The dog will also be microchipped for identity purposes and SABBR will be listed as the secondary contact. I also agree to always have the registration tag attached to the collar along with an identification tag stating the dog’s name and the owner’s contact number.

  4. If I can no longer care for the dog listed on this adoption agreement, I agree: a. to inform SABBR of this and either;
  5. return the dog to SABBR or,
  6. with SABBR’s prior approval, re-home the dog to a new owner.
    b. If the dog has been taken outside of the Perth metropolitan area (defined in the Planning and Development Act 2005) after adoption, I will arrange for the dog’s return to SABBR and any costs incurred to return the dog to SABBR will be at my expense.

  7. If I have adopted a ‘bonded pair’ (two dogs that must be homed together) and I can no longer care for the either one or both dogs listed on the adoption agreement, I agree to inform SABBR of this and return both dogs to SABBR.

  8. I agree that it is my responsibility to ensure that the dog listed on this adoption agreement is well looked after, has access to daily nutritional food, fresh water and shelter, which will always be available in accordance with the Animal Welfare Act 2002.

  9. I agree to keep the dog listed on this adoption agreement in good condition and healthy (both physically and mentally), including but not limited to regular bathing, grooming, worming, flea treatment, heartworm treatment and vaccinations as advised by a veterinarian.

  10. I agree that I will not neglect, mistreat or abandon the dog listed on this adoption agreement at any time.

  11. I agree that I will seek immediate veterinary advice and/or treatment if the dog listed on this adoption agreement becomes ill, injured, behaviorally challenging or develops a disease.

  12. I understand that the dog listed on this adoption agreement has been checked by a veterinarian and any further treatment required is to be the responsibility and at the expense of the new owner as stated on this form.

  13. I agree that the adopted dog listed will not be housed on a permanent basis with another dog of the same sex unless there is a minimum of six years age gap between the two unless authorised by the SABBR committee and documented below in the other additions to this agreement.

  14. I understand that I take the dog listed on this adoption agreement at my own risk and SABBR does not accept responsibility for any illness or injury following the adoption of the dog listed on this agreement. SABBR makes every effort to ensure that the dog is free of known disease, illness, injury and behavioral problems and that new owners are adequately informed of any concerns where possible.

  15. I acknowledge that if the dog listed on this adoption agreement is under the age of 18 weeks that puppy school is recommended to develop their social skills. If SABBR deems a dog must be enrolled in puppy school, this will be noted in the ‘Other additions to the agreement’ section.

  16. I certify that all the information I have provided to SABBR as part of the application and adoption process is true and correct. I understand and agree that any willful dishonesty that SABBR becomes aware of (whether verifiably or not to yourself) can and will result in termination of this agreement. If SABBR believes that the misleading information provided is likely to negatively impact the dog listed in this adoption agreement, SABBR reserves all rights to repossess the dog listed in this adoption agreement immediately with warning being given in person at the time of recovery.

  17. I understand that SABBR reserves all rights to immediately repossess any dog listed in this adoption agreement, without warning, if they become aware of any mistreatment of the dog listed in this adoption agreement in accordance with the Animal Welfare Act 2002.
